Job Description:

Lutheran Social Services of Wisconsin and Upper Michigan (LSS) seeks a motivated and compassionate Case Coordinator to join our dynamic team dedicated to fostering health, healing, and hope in the community. The Case Coordinator will play a critical role in managing and delivering comprehensive support services tailored to individuals and families facing a variety of life challenges. This position demands a blend of direct client interaction and administrative acumen, ensuring that all aspects of care coordination are executed with sensitivity and efficiency. Evaluate client needs and develop comprehensive service plans. Coordinate and monitor services provided by internal teams and external agencies. Maintain accurate and timely documentation of all client interactions and progress. Conduct regular follow-up meetings to assess client progress and update service plans as necessary. Build and maintain positive relationships with clients, families, and service providers. Attend and contribute to multidisciplinary team meetings. Ensure compliance with all federal, state, and agency regulations regarding social service provisions. Strong interpersonal and communication skills. Ability to manage multiple cases efficiently. Proficient in case management software and record keeping. Understanding of mental health issues and social dynamics. Crisis intervention abilities. Medical Insurance Dental Insurance Vision Insurance 401k Certified Social Work Case Manager (C-SWCM) preferred but not required.
