Evaluate patients' functional status and occupational therapy needs to develop appropriate treatment plans. Implement occupational therapy interventions based on patient needs and goals. Document patient progress and adjust treatment plans accordingly. Educate patients and families on home health techniques and goals. Communicate regularly with a multidisciplinary team to ensure comprehensive patient care.
