Schedule: 100% FTE, day shift working Monday - Friday, 8a - 5p. You
will work at University Hospital in Madison, WI. Be part of
something remarkable As an Advanced Practice Provider, you'll play
an integral role on our team by providing high-quality,
cost-effective care to patients. We are seeking a Nurse
Practitioner or Physician Assistant to: - Provide complex symptom
management recommendations. - Work in collaboration with the
Palliative Care Medical Director, attending medical team, and
Palliative Care Team members to deliver palliative care to patients
in the acute care setting with life-limiting chronic diseases. -
Effectively communicate with patients and family by utilizing
concepts of motivational interviewing. - Perform assessment process
to include patient history, physical, psychosocial, and spiritual
assessment. - Provide support, counseling, and information to
patients and families dealing with end-of-life issues or with
issues related to life-threatening or serious and complex illness.

Education: Minimum – If Nurse Practitioner: Master’s or Doctorate’s
degree from accredited Nursing Program. If Physician Assistant:
Bachelor’s or Master’s degree from accredited PA Program. Licenses
and Certifications: Minimum - If Nurse Practitioner: Active RN and
APNP license in State of Wisconsin AND active board certification
as a NP in Adult; Adult/Gerontology Acute Care; Adult/Gerontology
Primary Care. If Physician Assistant: Active PA license in State of
Wisconsin AND active PA board certification. All Nurse Practitioner
and Physician Assistant applicants must have: Active DEA license or
ability to apply for such license prior to or upon hire AND CPR
certification and advanced life support certifications if relevant
