Occupational Therapist- Home Healthcare
As a Occupational Therapist (OT) in home healthcare with AssureHHI,
you shall provide services that help restore function, improve
mobility, relieve pain, and prevent/minimize permanent physical
disabilities of patients suffering from injuries or disease. A
successful Physical Therapist teaches and trains patients and
caregivers to be an active participant in their recovery to
restore, maintain, and/or promote overall fitness and health. Their
patients include accident victims and individuals with other
disabling conditions such as low back pain, arthritis, heart
disease, fractures, hip and knee replacements, head injuries,
strokes, and other neurodegenerative diseases and gait
disorders.
Responsibilities:
Implement occupational therapy services in accordance with
physician's plan of treatment.
Assist the physician in the evaluation and assessment of the
client's level of
functioning by applying appropriate tests and assist in the
development and revision of the plan of care. Treat the client
through use of therapeutic activities designed to restore function
and self care activities for improving function. Observe record and
report the clients' reaction to treatment and any changes in the
clients' condition to the physician, Director of Nurses and Case
Manager. Educate client/caregiver and health care team in the
phases of occupational therapy in which they may assist the client,
in the use of adaptive devices, and durable medical equipment, as
appropriate. Provides staff in-service education as directed by
DON.
Assess and regularly re-evaluate the needs of the client, develop,
implement and revise the client's plan of care.
Initiate diagnostic, preventive and rehabilitative procedures as
appropriate to the client's care safety. Assist with plan of care
and maintain continuity of multidisciplinary clients whenever
needed to ensure good coordination of care. Promote coordination of
care through interdisciplinary case conferences and communication
with physician and field staff.
Develop, prepare and maintain individualized client care progress
records with accuracy, timeliness and according to policies. Submit
accurate documentation in accordance with agency policies.
Keep abreast of occupational trends and knowledge for service
provision, documentation and care coordination.
Participate in staff meetings, in-service programs and Utilization
reviews.
Participate in the agency's quality improvement program.
Supervise the Occupational Therapy Aide at least every 30 days
Qualifications:
Must be a licensed Occupational Therapist according in the state of
Illinois.
Graduated after successful completion of an Occupational Therapist
education program accredited by the Accreditation Council for
Occupational Therapy Education (ACOTE) of the American At least 2
years of experience as a Occupational therapist, some experience in
home health or public health nursing preferred. Proficient in
utilizing technologies including Microsoft Word, Excel, PowerPoint,
Outlook, Teams, SharePoint, and other software related to role and
responsibilities. Hold a valid driver's license, own a dependable
vehicle, and maintain a clean driving record.
Skills and Abilities: Demonstrate a passion for client service,
particularly in supporting individuals with disabilities and those
in need of supportive or senior care. Capable of building rapport
and establishing trust with existing clients, referrals, potential
clients, professional networks and communities, as well as
colleagues. Exhibit exceptional customer service and organizational
skills.
Keen attention to detail and accuracy in documentation of client
records.
Excellent organizational skills and ability to prioritize tasks in
a fast-paced environment.
Effective communication skills to interact with clients, client
families, and other care team members. Maintain confidentiality of
matters related to patient care and the agency business in
accordance with HIPAA rules and regulations. Maintains
professionalism in implementing therapy care and establishing
rapport with patients. Treat patients with respect and dignity.
Demonstrates autonomy, organization, assertiveness, flexibility,
and cooperation in performing job responsibilities.
Exceptional organizational aptitude with the ability to prioritize
tasks effectively in a fast-paced environment.
Exceptional communication skills, both written and verbal, as well
as during presentations.
