Job Description QUALIFICATIONS: Education : Minimum qualifications include a Bachelor degree from four-year college or university in Human Services (including Education), Behavioral Science, Social Science or health related field Experience: One year of specialized training or experience in the field of disabilities is preferred. Skills : Demonstrate the ability to plan, prioritize and organize tasks effectively. The Service Coordinator (SC) must be able to work independently with minimal supervision. The SC has strong interpersonal skills and must be able coordinate the supports and services for children and their families, focusing on satisfactory resolution of concerns. The SC has knowledge of child development and developmental milestones. The SC must be able to complete all paperwork in a timely/ accurate manner as well as meet all deadlines in compliance with program rules and responsibilities. The SC must demonstrate proficiency in Microsoft Office software applications and is able to adapt to new and updated software applications. Other : The position requires the use of the employee---s personal automobile, valid driver license and the minimum amount of auto liability insurance; ability to work flexible hours. Physical Demands: Must be able to drive, access homes and apartments by stairs, speak with and hear people. (Currently due to the pandemic, client home visits are not required, meetings take place via virtual platforms) JOB SUMMARY : Service coordination is provided to families with children, age 0 to 3, who have disabilities, and/or delays. The Service Coordinator provides families information and referrals, understanding of developmental milestones, evaluation/assessment, eligibility determination, family service plan development, arrangement of services, and transition into the local education agency. The Service Coordinator provides family support while upholding the federal and state guidelines the Department of Human Services Bureau of Early Intervention requires CORE COMPETENCIES 1. Knowledge and understanding of child development and developmental milestones SPECIFIC RESPONSIBILITIES: 1. Provide accurate information and referral information to individuals seeking service. 2. Initiate contact with family and gather information required by Early Intervention system about child and family 3. Connect families to evaluations to determine individual need and eligibility for program 4. Develop Individual Family Service Plan with families. 5. Coordinates therapy services with families and monitors service delivery in line with the identified needs of the child and family. 6. Travel to locations convenient for the families. 7. Maintain accurate and timely client record keeping and service reporting based upon the Child and Family Connection Procedure Manual. 8. Establish and maintain cooperative working relationships with all service provider agencies. 9. Prepare for and perform six (6) month and annual review meetings. Prepare for and facilitate service change requests, transition meetings with school districts and exit interviews. 10. Participate in agency informational, planning and management meetings. 11. Attend training events to maintain SC credentialing and enhance skills. 12. Perform other duties as assigned or required.
